If you’re a new patient, you can save time during your first appointment by completing our Patient Intake Form. Simply print out the Patient Intake form, fill in the information requested, and bring the completed form with you to your appointment. Please be sure to complete this paperwork BEFORE your scheduled visit, or arrive at least 45 minutes before your scheduled time to complete the questionnaire at our office. We’ve reserved a full hour for your initial appointment, so don’t let incomplete paperwork take away from the time set aside for you!
